ERECTIAL DYSFUNCTION FAQ’S
We’re sure you have questions, and we’re here to answer them! Below are some frequently asked questions that might help, but we’re also available via phone or email for anything that might be on your mind!
The persistent or recurrent inability to achieve or maintain an erection good enough to complete your chosen sexual activity satisfactorily, whether that’s masturbation, oral sex or vaginal or anal intercourse. The occasional inability to achieve a satisfactory erection doesn’t normally constitute a problem.
To diagnose erectile dysfunction, our doctors use a verbal questionnaire and a physical exam. However, if you have underlying health conditions, you might need a blood test or urine test to get an official diagnosis. Further testing, like a penile doppler ultrasound, might be performed to assess blood flow in the penis, providing critical information on vascular health. Additionally, reviewing your medical history can highlight potential contributing factors, such as medications or psychological conditions, essential for developing an effective treatment for erectile dysfunction at our ED clinic.

Erectile dysfunction affects men from various backgrounds and for a multitude of reasons. While age itself isn’t a cause of erectile dysfunction (ED), the risk nevertheless increases as you get older: 18% of 50-59-year-olds have trouble with their erections compared with 7% of 18-29-year-olds. However, it’s also possible for symptoms to start earlier. If you’re worried you’re starting to experience symptoms, reach out to a professional at our men’s erectile dysfunction clinic.
- There are two main causes of ED: physical and psychological. Most doctors agree that the majority of cases are physical but it’s also clear that many men with ED also quickly start to feel anxious, stressed or depressed. These feelings can easily make the symptoms of ED worse.
The main physical causes are:
- Diabetes. Up to 25% of all diabetic men aged 30-34 are affected by ED, as are 75% of diabetic men aged 60-64.
- Inadequate blood flow to the penis because arteries have got furred-up (a condition called atherosclerosis) or damaged. This causes about 40% of ED cases in men aged over 50. Smoking cigarettes, which is implicated in up to 80% of ED cases, constricts the blood vessels and is a major cause of damage to the arteries leading to the penis. One little-known cause of damage to key blood vessels is cycling.
- Regular heavy drinking. Alcohol can damage the nerves leading to the penis, reduce testosterone levels and increase levels of the female hormone estrogen.
- The side-effects of prescribed drugs, particularly those used to treat high blood pressure, heart disease, depression, peptic ulcers, and cancer. As many as 25% of ED cases may be caused by drugs taken to treat other conditions.
- Spinal cord injury. Almost a quarter of men with spinal injury are affected by ED.
- Prostate gland surgery (or other surgery around the pelvis). The risk of ED depends on the type of surgery, but up to 30% of men who have a radical prostatectomy (the complete removal of the gland affected by cancer) will experience ED.
The main psychological causes of ED are:
- Relationship conflicts
- Stress and anxiety
- Depression (90% of men affected by depression also have complete or moderate ED)
- Unresolved sexual orientation
- Sexual boredom
One way of working out whether your ED has a physical cause is to see whether there are any circumstances in which you get an erection. If you can produce one when masturbating but not with a partner, wake up with an erection, or have erections during the night, then there’s a good chance that your ED has psychological causes.
Erectile dysfunction doesn’t last for a specific period of time and its duration often depends on the cause. Some men experience temporary ED due to stress, anxiety, or certain medications, which may be resolved with lifestyle changes or addressing the underlying cause. Chronic or long-term ED might continue due to health conditions like diabetes, cardiovascular disease, or hormonal imbalances. Some ED clinic patients may experience symptoms for years, while others find that their symptoms subside after a few weeks or months with the right ED solutions..
Erectile dysfunction isn’t necessarily permanent because it can be the result of various factors such as stress, medication side effects, health conditions, or lifestyle choices. Most cases of ED are treatable with lifestyle changes, medication, therapy, or medical procedures. ED treatments will depend on the underlying cause, so it’s important to consult a professional for help. With proper intervention, the right ED solutions, and the guidance of our erectile dysfunction clinic, many men experience improvement in their symptoms.
Improving overall health can help prevent erectile dysfunction. Quitting smoking and drinking alcohol in moderation will help. Regular aerobic exercise and a low-fat diet will also reduce the risks of atherosclerosis. If you have diabetes, ensure it’s properly controlled.
– Yes. Get help and advice as soon as you notice a problem. This isn’t only important in terms of getting treatment for your ED: your struggle could also be a symptom of other potentially serious conditions (such as diabetes or heart disease).
One study of 50 men with ED who had sought prescriptions for Viagra found that although none of them had any symptoms of heart disease, six were found to have blockages in all three major heart arteries, seven had two arteries that showed narrowing and one artery was blocked in another seven. Overall, 40% of the men were at significant risk of angina or a heart attack.
If you are on treatment for a medical or psychiatric problem and you think that this treatment is affecting your sex life, tell your doctor. Alternative treatments that may have less likelihood of affecting sexual function are often available.
If your doctor doesn’t take your problem seriously, ask to be referred to a specialist. Don’t let yourself be fobbed off with comments like, “What do you expect at your age?”
At Golden State Urology, we offer personalized solutions that go beyond prescription drugs. Our erectile dysfunction treatment clinic ensures that your erectile dysfunction care is tailored to you.
Yes, seeing a urologist for ED can help you diagnose and treat the problem. Urologists specialize in male reproductive and urinary health, offering expertise in addressing ED through medication, lifestyle changes, or procedures.
There are ED surgery options available for patients with severe cases. One option is a penile prosthesis, which is a small device that is inserted into the penis and can help it create an erection. Patients who have experienced ED as a result of traumatic injury can also undergo microsurgical penile revascularization as an ED solution. This procedure connects the blood vessels in the penis and lower abdomen, increasing blood flow to the reproductive system.

CIRCUMCISION REVISION FAQS
We’re sure you have questions, and we’re here to answer them! Below are some frequently asked questions that might help, but we’re also available via phone or email for anything that might be on your mind!
In general, it is similar to recovery after circumcision. In our practice, most men are able to resume work within 1-3 days after circumcision revision. Many men do not require pain medications beyond over-the-counter anti-inflammatory pain relievers.
Circumcision revision is a surgical procedure performed to correct or improve the appearance of the penis and penile skin after a prior circumcision. The most commonly performed revision is straightening of the circumcision scar or removing the excessive or uneven foreskin.
A more complex surgical procedure may be required in some men to correct the appearance of the penis after prior circumcision. This may require using advanced plastic surgery techniques such as flap rotation or grafting skin from other areas.
Revision of circumcision is a sometimes necessary procedure for those who developed issues and complications from the original neonatal or adult circumcision. This can be done when the doctor or urologist failed to remove enough foreskin leaving too much of it left, if the penis drops back to the fatty area around it (buried penis), or if adhesions or penile skin bridge develop.
It’s never late to repair or correct incomplete circumcisions in older boys and even circumcised adult men. No matter how long has passed since the original circumcision, problems like these won’t fix themselves and could even worsen over time. So if you suspect that something might be wrong, absolutely contact your doctor or urologist, or visit a hospital or your local male health clinic.
If you’re looking into getting a circumcision revision, contact or book an appointment with Golden State Urology.

VASECTOMY FAQS
We’re sure you have questions, and we’re here to answer them! Below are some frequently asked questions that might help, but we’re also available via phone or email for anything that might be on your mind!
Vasectomy is a form of male sterilization, or “permanent contraception”. In this procedure, the vas deferens (sperm duct) is either blocked or partially removed on both sides. Thus, sperm from the testes no longer reaches the ejaculate, and no longer enters the female reproductive tract to cause pregnancy.
The patient is positioned on an exam table, and the scrotal skin is cleaned. On each side of the scrotum, the site of interest is numbed with local anesthetic, and a small (3-5mm) opening is made in the skin (by incision or puncture). Here at Hopkins, we perform the “no-scalpel” puncture technique, as this heals well without stitches and may be associated with less bleeding.
The vas deferens is gently separated from surrounding tissues, then cut and sealed. A small segment of the vas deferens is often removed as well. All structures are then returned to their normal position within the scrotum. The skin incisions are so small that stitches may not be used. If used, the stitches will dissolve on their own. Some ointment and/or gauze may be placed on the wounds.
Typically, the procedure itself takes around 15-30 minutes. However, the office visit will be longer due to the time necessary to get positioned and cleaned for the procedure. If general anesthesia is administered in the operating room, this can add a few hours of recovery time as well.
It is greater than 99%. In rare instances, the sperm can reform a path between the ends of the vas deferens. This is called recanalization, and leads to pregnancy in 1 out of every 2000-3000 patients that get a vasectomy. Vasectomy is still considered among the most effective and safe forms of permanent contraception.
Patients should consider vasectomy permanent.
If your objective is merely to space out pregnancies, or if you have even the slightest reason to believe that you might want to have children in the future, then a vasectomy will not suit your purpose and should not be considered.
Cryopreservation (sperm banking or freezing) before vasectomy is available as an option for those men considering vasectomy, though they should understand that use of the sperm requires professional assistance. Although we do not recommend sperm banking for all couples, some couples may elect to pursue this. Vasectomy reversal is also an option, but should not be taken lightly, as this can be costly and is not 100% effective.
You probably won’t need to wait long before you can return to normal activities after vasectomy.
A vasectomy is an outpatient procedure in which your surgeon cuts and closes off the tubes that deliver sperm from your testicles into your semen. Most vasectomies can be done in a urologist’s office. The procedure itself is quick, taking about 30 minutes or less.
Full recovery time is about eight to nine days for many people. Keep in mind this may vary depending on your individual perception of pain and ability for tissue healing.
It will take longer until you can ejaculate without sperm in your semen.
There are risks for every surgery. A sperm granuloma occurs in 15 to 40 percent of men who undergo vasectomy. A sperm granuloma is a mass that develops over time as a result of the body’s immune reaction to sperm leaking from the cut end of the vas. It is typically treated with an anti-inflammatory medication, such as ibuprofen. Post-vasectomy pain syndrome – This condition is thought to result from a buildup of fluid in the epididymis leading to a chronic dull ache in the testes.
Apply ice packs intermittently to the scrotum the night of your vasectomy and as much as possible the following day. Do not apply for more than 20 minutes at a time. Remove all dressings–except one or two clean gauze pads–from inside the athletic supporter after 24 hours.
As partners, you may need to support your partner after the surgery.

MonaLisa Touch is also suitable for women who wish to improve vaginal health without hormone-based treatments. Ideal candidates are those seeking a minimally invasive procedure to address these symptoms and restore comfort and elasticity to the vaginal tissue.
The MonaLisa Touch laser treatment works by using a CO2 fractional laser to gently stimulate and heat the vaginal tissue and blood vessels, promoting collagen production and tissue regeneration. This process helps restore elasticity, vaginal moisture, and overall health to help alleviate symptoms like dryness and discomfort. The treatment typically involves minimal downtime.
The MonaLisa Touch® is an in-office procedure performed by an experienced physician. The procedure is virtually painless and requires no anesthesia. Some patients may experience some discomfort the first time the probe is inserted, but the actual treatment is not painful. A consultation with a MonaLisa Touch laser treatment doctor can answer any questions about the discomforts involved and how to manage them.
The procedure is similar to almost any other vaginal rejuvenation procedure and is not as invasive as plastic surgery. A strong lidocaine-based numbing cream will be applied prior to the procedure. This will ensure that this is a painless process. Before the laser is activated, everyone in the room must wear special eye protection which will be provided. The metal wand used will feel cool initially inserted. You will feel a vibrating sensation. You may experience a tingling sensation and or air puffs during the treatment. These may get stronger near the vaginal opening. There might be a slight sizzle sound during the laser pulse. There might be a slight odor from the vaginal tissue being warmed up. The procedure lasts about 5-10 minutes. An occlusive ointment will be applied after the completion of the procedure. A pad will be provided to protect your clothing. There will be no dermal fillers or chemical peels involved in this.
Some women experience a watery discharge or light spotting for a day or two after the cosmetic surgery. The vaginal area may feel swollen or full. Do not put anything inside your vagina for 48 hours such as tampons or sexual intercourse. After 48 hours, the area should feel normal with no discomfort. You will be scheduled for 2 additional treatments 6-weeks apart to complete the recommended protocol to achieve the maximum benefits from the MonaLisa Touch® procedure.
- Your skin may feel sensitive and may be red and swollen. The treated area may be itchy.
- Use cold compresses or cold gel packs (20 minutes on, 20 minutes off) as needed for swelling and discomfort.
- Gently cleanse the area, including the labia minora and labia majora, 2-3 times a day with mild soap.
- Dry the area with a hair-dryer on low heat instead of wiping with a towel. This is much less abrasive to the tissues. And a better way to improve the results.
- Reapply a skincare occlusive ointment (Aquaphor) after each washing and continue to use until your skin is healed.
- You use Motrin or Advil as needed.
- Avoid lifting heavy weights or intense physical exercise for 72 hours following the treatment.
- Wear loose, cotton underwear.
- Avoid wearing pantyhose and tight-fitting pants.

THE UROLIFT SYSTEM FAQ’S
The FDA-cleared UroLift System is a relatively new treatment for BPH, and it’s the only treatment that uses the Prostatic Urethral Lift (PUL) procedure. The UroLift System treatment is a proven, minimally invasive approach for treating lower urinary tract symptoms due to benign prostatic hyperplasia (BPH) that can get men off BPH medications and avoid major surgery. The UroLift System’s permanent implants, delivered during a minimally invasive outpatient procedure, relieve prostate obstruction and open the urethra directly without cutting, heating, or removing prostate tissue.
As with any medical procedure, individual results may vary. Speak with your urologist to see if the UroLift System treatment is right for you. Most common side effects are mild to moderate and include pain or burning with urination, blood in the urine, pelvic pain, urgent need to urinate and/or the inability to control the urge. Most symptoms resolve within two to four weeks after the procedure.

The UroLift System consists of a delivery device and tiny permanent implants.
- The UroLift Delivery Device is placed through the obstructed urethra.
- Small UroLift Implants are placed to lift or hold the enlarged prostate tissue out of the way. The Implants are delivered through a small needle that comes out of the delivery device and into the prostate.
- The UroLift Device is removed, leaving an open urethra that provides symptom relief.
One unique aspect of the UroLift System is that it does not preclude retreatment or BPH treatments such as TURP or laser, should they be required in the future.
The UroLift System is a proven treatment option for BPH patients who are looking for an alternative to drugs or major surgery. You may be considering or scheduled for a TURP/Laser treatment, or are unhappy with or have stopped taking medications. The UroLift System is appropriate for patients seeking a minimally invasive treatment and are concerned about preserving their sexual function and their quality of life.
If you and your doctor decide that the UroLift System treatment is right for you, your doctor will provide the specific, detailed information relating to your condition. In general, the UroLift System is a minimally invasive treatment that entails minimal downtime. Your doctor will use the UroLift delivery device to deploy permanent implants to relieve obstruction caused by the enlarged prostate that is pressing on your urethra. The procedure may be performed under local or general anesthesia and you may be given medication to feel comfortable during the treatment. Typically, no catheter and no overnight stay is required post-treatment.
After the treatment, almost all patients go home the same day without a catheter. There is minimal downtime post-treatment and many patients experience symptom relief in as early as 2 weeks. Patients may experience some urinary discomfort during the recovery period. The most common side effects may include light blood in the urine, some pain or discomfort when urinating, some increased urge to go and discomfort in the pelvis that typically resolve within two to four weeks after the procedure.
Sexual function has been preserved among the hundreds of patients treated in our clinical studies.1,2,3,4 This is a unique benefit of the UroLift System treatment compared with other BPH therapies such as TURP, laser, and even medication.
The primary contraindications are an obstructive or protruding median lobe of the prostate and a prostate with a volume of >80cc.
Non-clinical testing has demonstrated that the UroLift Implant is MR Conditional. That means that you can be safely scanned in an MR system meeting the following conditions:
- Static magnetic field of 3 Tesla or less
- Maximum spatial gradient magnetic field of 1500 Gauss/cm (15 T/m) or less
- Maximum MR system reported, whole body averaged specific absorption rate (SAR) of 4 W/kg (First Level Controlled Mode)
Under the scan conditions defined above, the UroLift implant is expected to produce a maximum temperature rise of 2.4°C after 15 minutes of continuous scanning.
In non-clinical testing, the image artifact caused by the device extends approximately 15 mm from the UroLift Implant when imaged with a gradient echo pulse sequence and a 3 Tesla MRI system.
Patient implant cards are provided to inform the patient that the UroLift implant is MR Conditional and can safely be scanned only under specific MR conditions.
If you have any questions about MRI safety, please consult your doctor.
Over 950 patient-years of clinical data has been presented in peer-reviewed publications to date demonstrating that the UroLift System is safe and effective. Clinical studies have been conducted in the United States, Canada, Europe and Australia.
No. The implants are placed on the anterior (front) side of the prostate, and a DRE is conducted on the posterior (back) side of the prostate. The implants are not expected to interfere in a DRE.
The first step is to make an appointment with your urologist to determine if you are an appropriate candidate for the treatment. Your physician’s office staff can work with you and your health plan to learn more about your healthcare benefits specific to the UroLift implant procedure.
Yes, all Medicare carriers provide benefits for the UroLift System when medically necessary. Discuss the UroLift System with your physician to determine if the UroLift treatment is an option for you.
UroLift is a safe, minimally invasive solution for treating BPH. By helping reduce obstructive prostate tissue, UroLift can help relieve symptoms of BPH without interfering with sexual function. Side effects are minimal and the procedure is quick to complete.
The Implants are intended to be permanent. The implant is made up of standard surgical implantable materials: a nitinol capsular tab, a stainless steel urethral tab, and polyethylene suture that holds the two tabs together. Your doctor can remove the urethral implant, if needed. The suture can be cut and the urethral endpiece can be retrieved with a standard grasper. The capsular tab will remain in place inside the body.
A urologist, a doctor who specializes in the treatment of diseases of the urinary system and sex organs, uses the UroLift System to treat urinary symptoms due to BPH. Comprehensive training is provided to the urologist prior to using the UroLift System.
NeoTract, Inc. manufactures the UroLift System. The company is based in Pleasanton, California.

Typically performed under general or local anesthesia for an outpatient procedure, UroLift recovery is a relatively fast process. You’ll need to stay home and rest for two to three days after the procedure. During this time, you may experience a burning sensation or blood in your urine, but this will subside as you heal. You’ll also need to avoid intense exercise, sexual intercourse, and other strenuous activities for two to three days after the procedure.
